Asthma Action Plan

Everyone with asthma should have an Asthma Action Plan in writing. This plan provides information and instructions on how you can manage your asthma.

It includes:

  • Medicines
  • Recognizing when your symptoms get worse
  • What to do in an emergency

If you do not have a written Asthma Action Plan, take the form below to your doctor and have them fill it out for you.
